Overview
Your role will be to provide information and support to colorectal cancer patients from the point of diagnosis throughout their surgical and oncological management and living with and beyond cancer into their survivorship. You will be a valued member of the multidisciplinary team, promoting the best patient experience possible.
The post holder will provide specialist, high-quality, expert nursing care and support for patients and their carers with a colorectal cancer diagnosis. You will deliver evidence-based, individualised patient care according to the patient\'s changing health care needs, including holistic needs assessment and responsive care planning (HNA). You will work autonomously and develop collaborative relationships with others across and within professional boundaries. You will work collaboratively to develop patient pathways and be responsible for the continuous review and development of the nursing service. The post holder provides expert knowledge and advice in their own specialist area, ensuring maintenance of clinical excellence underpinned by evidence-based practice. Working with the Multidisciplinary Team and Cancer Management Team, you will ensure that your cancer service complies with the national standards for safety and quality, assessed through the National Cancer Peer Review Process.

Responsibilities
* Provide specialist nursing care and support for patients with a colorectal cancer diagnosis, including carers.
* Deliver evidence-based, individualised patient care and holistic needs assessment (HNA) with responsive care planning.
* Work autonomously and develop collaborative relationships across professional boundaries.
* Collaborate to develop patient pathways and contribute to ongoing review and development of the nursing service.
* Offer expert knowledge and advice within your specialist area and maintain clinical excellence underpinned by evidence-based practice.
* Ensure cancer service compliance with national safety and quality standards, contributing to plans arising from Peer Review assessments.
* Promote wellbeing and safety by acting on concerns, communicating effectively, and sharing information per policy and procedures.
Requirements / Qualifications
* Experience in colorectal cancer nursing or a closely related area (in line with job level).
* Ability to work autonomously and within a multidisciplinary team.
* Understanding of holistic needs assessment and care planning.
* Commitment to evidence-based practice and continuous service improvement.
