The UCR Service provides urgent support, seven days a week, to help prevent unnecessary hospital admissions. This is achieved by providing a two-hour crisis response delivered by a multi-skilled team of professionals. Support is provided at home or where the patient usually resides for those concerned about declining health or mobility, putting them at risk of hospital admission.
Our teams work closely with local ambulance services to support category 3 & 4 patients, facilitating rapid community response by teams equipped to help patients remain at home. The UCR service also offers a virtual ward supported by digital technology and remote monitoring, ensuring hospital-level care at home in familiar surroundings, aiding recovery and freeing hospital beds.
Teams, comprising GPs, Nurses, Paramedics, Pharmacists, Occupational Therapists, Physiotherapists, and Health & Therapy Assistants, work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm. They review patients daily, with ward rounds conducted via home visits or video technology.
The service provides clinical interventions, including intravenous therapy, as part of a multidisciplinary team for admission avoidance pathways, in line with national specifications. The service triages, assesses, and treats patients over several days to ensure safe home care.
The postholder will offer clinical expertise, support team coordination, allocate visits, and contribute to service development through reflective practice, supervision, and audit. They will identify suitable patients for early discharge or admission prevention.
