Our PCN is looking for a physiotherapist for between 15 and 30 hours per week to join the PCN's wider MDT team. The PCN has its own building located in South Shoebury and covers 4 practices. The role will involve working autonomously in your own clinics with clinical support from other PCN clinicians. Shifts can be flexible with the option of working later in the evenings and Saturdays for enhanced rates of pay.
Main duties
To apply, you should be a fully HCPC and CSP registered physiotherapist with at least 5 years post-graduate experience, including 3 years of relevant MSK clinical experience in line with HEE's Roadmap to Practice. You should be driven, passionate about developing your clinical skills to become the best diagnostic clinician you can be, and flexible to the changing pace of primary care. You should be able to work to a 20 minute appointment timing. Identify patients that need a joint injection and refer to other clinicians if applicable. Work as part of a wider MDT in the PCN to deliver PCN projects such as the CVD LES.
About Us
Southend East PCN is located at Norton Place in Shoebury, along Ness Road. We have our own PCN building actively used by 5 member practices. We cover approximately 37,000 patients and offer a range of appointments from a wide range of staff, fully utilising the ARRS budget. Our team includes pharmacists, pharmacy technicians, social prescribers, health trainers, paramedics, physiotherapists and assistant practitioners. We have 2 clinical directors who are partners in two local surgeries, and an operations manager. We have a strong digital presence and have employed a Digital Transformational Lead to help our PCN network with other businesses and mature over time.

Job responsibilities
Clinical Practice
 * Assess, diagnose, triage, and manage patients, taking responsibility for a complex caseload. Progress and request investigations when trained (e.g., x-rays and blood tests) and referrals to facilitate diagnosis and choice of treatment.
 * Develop and use your full scope of clinical practice, including independent prescribing, injection therapy and investigation when trained.
 * Manage complex interactions, including working with patients with psychosocial and mental health needs, referring to social prescribing when appropriate.
 * Deliver programmes of supported patient self-management to facilitate behavioural change, improve physical activity and independence, and minimise pharmacological interventions.
 * Provide care tailored to individual needs, including the impact of clinical status on general health, well-being, employment status, function, activity, mobility and independence.
 * Support patients to set their own goals and be confident in self-management.
Evidence, Research, and Development
 * Develop relationships and a collaborative working approach across the PCN supporting the integration of pathways in primary care.
 * Work as part of an MDT in a patient-facing role to strengthen links for wider MSK services through clinical leadership, teaching and evaluation skills.
 * Support research and audit programmes to evaluate and improve the effectiveness of the First Contact Practitioner (FCP) service.
 * Streamline the pathway of care to ensure timely access to care.
Leadership
 * Lead, manage and contribute to service delivery.
 * Implement clinical governance for own practice, including regular audit, evaluation, supervision and training.
 * Communicate effectively with patients and carers about diagnosis, prognosis and treatment choices to support personalised care.
Facilitating learning
 * Provide feedback and support to other PCN clinical team members on referral criteria.
 * Critically reflect on own practice and continue to develop skills.
 * Work across the multi-disciplinary team to create and evaluate effective and streamlined clinical pathways and services.
 * Provide leadership and support on MSK clinical and service development across the PCN, with learning opportunities for the whole MDT within primary care.
 * Develop relationships across the PCN to support integration of pathways in primary care.
 * Encourage collaboration across the wider health economy and contribute to developing physiotherapy clinical services across the PCN.
 * Liaise with secondary and community care services as required to support management of patients within the PCN.
 * Support regional and national research and audit programmes to evaluate and improve FCP effectiveness, communicating outcomes and integrating findings into practice.
Person Specification
Qualifications
Essential
 * Fully HCPC and CSP registered practicing Physiotherapist.
Desirable
 * Completion of some level 7 MSc learning; qualified injection therapist and/or independent prescriber.
Experience
Essential
 * Minimum 5 years post-graduate physiotherapy experience; minimum 3 years of MSK clinical experience.
Desirable
 * Experience in first contact triage work; GP practice experience; EMIS/SystmOne experience.
ARRS Requirements
 * Undergraduate degree in physiotherapy
 * Registered with HCPC
 * Public liability insurance
 * Masters level qualification or equivalent specialist knowledge, skills and experience
 * Level 7 capability in MSK areas of practice or equivalent (advanced assessment, diagnosis and treatment)
 * Ability to operate at an advanced level of practice
