What You Will Do:
Conduct Comprehensive Assessments: Collaborate with patients and their families to deliver tailored assessments and create personalised care and support plans.

Collaborate with Multi-Disciplinary Teams: Work closely with healthcare professionals and external agencies to deliver integrated and person-centred care.
Ensure Legal Compliance: Adhere to relevant legislation, including the Care Act 2014, Mental Capacity Act, and other regulatory frameworks.
Promote Independence: Advocate for patients by empowering them to maintain their independence and improve their quality of life through effective support planning.
Skills and Qualifications:
Essential: Degree in Social Work with current Social Work England registration.
Experience: Previous experience working in a hospital discharge team or adult social care setting.
Knowledge: Strong understanding of the Care Act 2014, safeguarding procedures, and best practices in hospital discharge.
Skills: Excellent communication, analytical, and report-writing skills, with the ability to manage complex caseloads.
