Job Purpose
Nottinghamshire County Council is developing an all‑age Mental Health offer, alongside existing work for people with Learning Disability and Autism. This will involve strengthened multi‑disciplinary working with a variety of partners, promoting best practice, managing demand and responding to legislative change, with a focus on reablement. The role will facilitate hospital discharge, ensuring high‑quality, person‑centred support and shaping services for local people.
