Responsibilities
* Enhance patient experience by promoting independence and supporting patients to maintain and maximise functional ability.
* Encourage and assist patients with mobility, nutrition, hydration, and support participation in daily interactive activities.
* Support the delivery of high‑quality care to older adults with frailty needs, focusing on improving overall patient experience.
* Promote and maintain patient independence, encouraging participation in mobility, nutrition, hydration, and daily interactive activities.
* Assist patients with personal care, including washing, dressing, toileting, and maintaining dignity at all times.
* Support safe mobilisation using appropriate equipment following moving and handling guidelines.
* Monitor and report changes in patients’ condition promptly to the registered nurse or therapy staff.
* Carry out delegated clinical tasks such as observations, fluid/food monitoring, and documentation as required.
* Work collaboratively with the nursing team, therapies team, and wider MDT to achieve individualised patient goals.
Qualifications
* NVQ Level 3 in Health (or equivalent).
* Effective communication skills.
* Excellent team player, able to work independently when required.
* Willingness to work closely with both nursing and therapy teams to deliver high‑quality, patient‑centred care.
* Desirable ability to speak Welsh (Welsh and English speakers welcome to apply).
