Key Responsibilities
* Deliver high-quality nursing care in the community, empowering patients to maintain independence.
* Assess, plan, implement, and evaluate person-centred care plans.
* Collaborate with multidisciplinary teams, GP surgeries, and acute trusts.
* Assist in facilitating smooth hospital discharges and reducing readmissions.
Essential Qualifications
* NMC registration (valid and active).
* A minimum of 6 months of NHS experience as a Community Nurse.
* Eligibility to work in the UK.
