The Practice currently consists of 5 partners, 2 salaried GPs,3 nurses and a supportive admin and reception team who care for a list size of10,000 patients.

Weare a training practice and offer high levels of support and training in anyareas of development for GP registrars and indeed all staff.
Thepost-holder will manage a caseload that includes a wide range of health needsin a primary care setting, ensuring the highest standards of care for allregistered and temporary patients.
Main duties of the job
In accordance with the Practice timetable, as agreed, thepost-holder will make him/her-self available to undertake a variety of dutiesincluding surgery consultations, telephone consultations and queries, visitingpatients at home, checking and signing repeat prescriptions and dealing withqueries, paperwork and correspondence in a timely fashion
Making professional, autonomous decisions in relation topresenting problems, whether self-referred or referred from other healthcareworkers within the organisation.
This role is for 6 sessions per week working over three days, Monday, Wednesday and Friday. Morning and afternoon sessions.

Job Summary:
The post-holderwill manage a caseload and deal with a wide range of health needs in a primarycare setting, ensuring the highest standards of care for all registered and temporarypatients.
ClinicalResponsibilities:
In accordance with the Practice timetable, as agreed, thepost-holder will make him/her-self available to undertake a variety of dutiesincluding surgery consultations, telephone consultations and queries, visitingpatients at home, checking and signing repeat prescriptions and dealing withqueries, paperwork and correspondence in a timely fashion
Making professional, autonomous decisions in relation topresenting problems, whether self-referred or referred from other healthcareworkers within the organisation
Assessingthe health care needs of patients with undifferentiated and undiagnosed problems
Screeningpatients for disease risk factors and early signs of illness
In consultation with patients and in line withcurrent Practice disease management protocols, developing care plans for health
Providingcounselling and health education
Admittingor discharging patients to and from the caseload and referring to other careproviders as appropriate
Recordingclear and contemporaneous consultation notes to agreed standards
Collectingdata for audit purposes
Compilingand issuing computer-generated acute and repeat prescriptions (avoidinghand-written prescriptions whenever possible)
Prescribingin accordance with the Practice prescribing formulary (or generically) wheneverthis is clinically appropriate
Ingeneral the post-holder will be expected to undertake all the normal duties andresponsibilities associated with a GP working within primary care.
