Overview
NHS Greater Glasgow and Clyde is one of the largest healthcare systems in the UK, employing around 40,000 staff in a wide range of clinical and non‑clinical professions and job roles. It delivers acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when its regional and national services are included.
This fixed‑term post of one year is part of the Home First Response service based within Renfrewshire Health and Social Care Partnership (HSCP). The service provides a multidisciplinary team (MDT) approach to support patients identified with frailty within the Royal Alexandra Hospital (RAH), in community and with MDT to support discharge from hospital and prevent admission.
Key Responsibilities
* Provide hands‑on care and support within a rehabilitation ethos for patients with frailty.
* Work effectively within a community rehabilitation MDT team, supporting nursing and allied health professional (AHP) care plans, treatments and therapies.
* Assist patients to live safely at home and enable recovery across all health fields.
* Peripatetic working across acute and community settings to support the development of a 7‑day service.
Qualifications and Experience
* Experience within a care environment or working towards SVQ Level 3 or equivalent experience.
* Ability to work effectively within a community rehabilitation MDT team.
* A valid UK driving licence.
* Not eligible for sponsorship – requires right to work in the UK.
Working Conditions
Hours of work are flexible to cover core hours, seven days a week. Initially hours will be 9 am – 5 pm. The role is fixed‑term for one year.
