Band 6 Specialist Mental Health Practitioner - Swindon
An opportunity has arisen for a Mental Health Nurse, Social Worker or an OT to join the Swindon Recovery Team at Chatsworth House, Swindon, Wiltshire. Candidates must be a part of a professional registered body, have experience of working with a variety of mental health needs and have a passion for developing cutting edge and evidence-based practice.
The role focuses on providing effective interventions and treatments to people with severe and enduring mental health needs, and their carers. This involves enabling and assisting service-users to meet daily health, social care and wellbeing needs. Our aim is to empower and enable service-users to maintain or regain their independence in line with their personal recovery goals.
You can expect high levels of support and direction and to be actively involved in the models which develop within the area with service management support. We also offer a fast-paced and challenging working environment, good supervision structures and training and more opportunities to develop. A carer progression will be supported if identified in Appraisal.
Responsibilities
* To undertake the full range of activities required to deliver ongoing comprehensive mental health assessment for service users with severe and enduring mental health needs living in the community and in a range of settings
* To act as care coordinator for identified service users, also providing defined interventions to individuals on other workers caseloads.
* To contribute to the planning, delivery and evaluation of defined, therapeutic interventions as identified, in line with personal recovery plans, including service users who may be on other caseloads.
* In collaboration with service users and carers, be responsible for facilitating the development of comprehensive crisis plans, rapid access plans, advance statements etc., involving other agencies such as primary care where appropriate.
* To be responsible for maintaining own workload, on a day-to-day basis, ensuring that time is prioritised effectively, making full use of electronic resources such as diaries/scheduling.
* To be responsible for the protection of individuals from abuse and harm in line with local safeguarding policies and procedures
* To proactively participate in management, workload and clinical supervision in accordance with trust policy, taking personal responsibility for making appropriate arrangements.
