Job Overview
We are seeking an experienced support worker, who can drive. The candidate must be UK based with the right to work (not somebody looking for sponsorship).
Key Responsibilities:
* Provide practical and emotional support to individuals, helping them access and participate in community activities, appointments, medication administration, outings, college (if required), shopping, getting involved in activities they enjoy and daily living tasks.
* Assist clients with personal care (maintaining personal hygiene), mobility, and household tasks where required, ensuring their dignity and independence are maintained.
* Develop and implement ongoing individual support plans with the care coordinators tailored to each client's specific needs and goals, in collaboration with the client and their family.
* Offer guidance and support on issues such as budgeting, accessing benefits, and managing health appointments.
* Supporting to pursue new hobbies or interests as well as helping to gain employment (if required).
* Ensuring their rights are respected and they are able to express their needs and preferences.
* Work closely with other healthcare professionals, social services, and community organisations to ensure the best possible outcomes for clients.
* Record and maintain accurate, confidential documentation of client progress and incidents as required via your notes.
* Ensure a safe, supportive, and non-judgemental environment for clients, promoting respect and equality at all times.
* Attend regular training sessions and team meetings to stay informed about best practices and relevant changes towards your client.
* Maintain awareness of safeguarding issues and act in accordance with safeguarding procedures to protect vulnerable individuals.
Essential Skills and Qualifications:
•Native English and clear speech
•Previous experience in a care or support role, preferably in the community or social care setting.
* Knowledge of safeguarding protocols and an understanding of the challenges faced by vulnerable individuals.
* Strong interpersonal skills, with the ability to build rapport and trust with clients and their families.
* Ability to work independently and as part of a team, showing initiative and flexibility.
* A non-judgemental attitude with a commitment to promoting diversity and inclusion.
* Excellent communication skills, both written and verbal.
* Ability to handle sensitive and confidential information with discretion.
* A full UK driving licence and access to a vehicle (if required for the role).
Desirable Qualifications:
* Level 2 or 3 Diploma in Health and Social Care (or equivalent qualification).
* First aid or mental health first aid training.
* Experience working with specific client groups, such as people with mental health conditions or learning disabilities

***Candidate must be available immediately and be prepared to take on shadowing with the current support worker to understand the role.***
