About the Role
An opportunity has become available for a Social Worker to join the fast-paced and supportive Hospital Social Work Team at Princess Royal Hospital, West Sussex.
West Sussex County Council is introducing new ways of working in hospitals to respond to the needs of the adult population. The Hospital Social Work Team works within the Discharge to Assess model, responding within timescales to assess people recently discharged from hospital, and helping arrange any care and support needed in the longer term to maximise independence.

Salary: £39,862 to £42,839 grade 10 or 11 dependent on experience

As an experienced Social Worker, you will be accountable for delivering a statutory social work service. You will conduct proportionate strengths-based conversations, enable maximum participation for the individual, plan, undertake reviews, and carry out safeguarding enquiries. You will be responsible for staying up to date with key developments and ensuring that business change is implemented.
You will need to work collaboratively with individuals and health colleagues, build on good partnership working, and promote equality of opportunity. You should be able to manage conflicting priorities while remaining resilient. You must stay up to date and compliant with relevant legislation, adhere to organisational procedures, policies, codes of conduct and the values of West Sussex. You will work with complex cases within the timescales of the Discharge to Assess Model and support the duty function on a rota basis. Clear English communication with members of the public is required.

2. Good communication skills to influence in a multi-disciplinary setting and explain complicated information; able to negotiate and encourage others to adopt a course of action.
3. Ability to maintain clear and accurate case recording and up-to-date casework on the West Sussex database.
4. Ability to think creatively and adapt to changing situations to tailor advice.
5. Ability to work proactively as part of the team to assess needs, negotiate and implement effective plans while prioritising workload and deadlines.
6. Demonstrate confident and effective judgement about risk and accountability in decision making.
