Responsibilities
1. Assist in the development, review, & revision of the plan of care.
2. Provide nursing care in accordance with physician’s plan of treatment and individual plan of care, as authorized by the client’s payer source.
3. Observe, record & report reactions to treatment & any changes in the client’s condition to appropriate personnel and/or physician.
4. Provide ongoing instruction & supervision of client, family, or other care team personnel in the plan of treatment.
5. Monitor & record patient's condition & document provided care services via electronic charting.
6. Participate in education programs, quality assessment activities, nurse’s meetings & other staff meetings as required.
Requirements
1. Active Licensed Practical Nurse or Registered Nurse License
2. Driver's License & Vehicle with Current Auto Insurance
3. Ability to Pass Drug Test, Background Check, & Physical
4. Problem-solving skills and ability to multi-task
5. Excellent Communication Skills
