Clinical Lead Community Nursing
We are looking to recruit an enthusiastic Clinical Lead to join the Rural Alliance Care Community Team in rural western Cheshire. You will be part of a multidisciplinary team comprising the Team Manager, Enhanced Clinical Practitioner, Clinical Leads, Community Nurses, AHPs, and other specialist professionals.
Responsibilities include:
* Provide a highly visible clinical profile with leadership and clinical oversight of a designated community caseload.
* Collaborate with the wider health, social care and third sector partner agencies to meet the needs of patients.
* Ensure patients receive the right care by the right person at the right time and place.
* Place the patient at the centre of decisions, reinforcing streamlined care and promoting independence.
* Guide the caseload in relation to current health and social care agendas and local initiatives.
* Be aware of local public health profiles and how they influence care needs.
* Focus on delivery of person‑centred care and provide evidence‑based care to a defined population through case load management and monitoring of standards.
* Provide clinical leadership.
Qualifications
* First‑level registered general adult nurse.
* Evidence of teaching and assessing.
* SPC qualification in District Nursing or equivalent.
Knowledge and Expertise
* Proficiency in delivering high‑quality evidence‑based care.
* Demonstrated continuing professional development.
* Effective communication and teamwork within an MDT setting.
Experience
* Significant experience within a community healthcare setting.
* Experience of making autonomous decisions.
