About the Role
We are seeking motivated and organised Care Navigator to support our clients Clinical Services and Urgent Care teams. This role is central to ensuring patients receive high-quality care and a seamless experience. You will provide a wide range of administrative support and act as a key point of contact for both patients and healthcare professionals.
This post is non-patient facing.
Key Responsibilities
* Deliver high-quality administrative support, including appointment booking and referral processing
* Use SystmOne to manage patient data accurately and confidentially
* Respond to patient and professional enquiries in a professional and compassionate manner
* Support the coordination of care across services and follow up on patient needs
* Work flexibly across teams to support service delivery
What We’re Looking For
Essential:
* Level 2 English and Maths (or equivalent experience)
* Excellent attention to detail and accuracy
* Strong organisational and administrative skills
* Excellent verbal and written communication
* Confident using Microsoft Office applications
* Ability to work independently and as part of a team
* Experience in the NHS or general practice
* Familiarity with SystmOne and referral systems such as Gateway
