Are you the special person who could fulfil the role of a Learning and Disabilities Practitioner, giving families a short break to children and young people aged 0 to 18 years with a disability or complex health needs?
This is a rewarding and exciting opportunity to join an experienced and dedicated team of practitioners working across Lincolnshire, providing the Children with Disabilities Support Service projects.
The services offer:
1. Early Support Learning Provision for children aged 0-5, providing social and learning opportunities on weekdays from a Children Centre or Family Hub in Skegness and Binbrook.
2. Targeted Positive Activities service for children and young people aged 5 to 18, offering community and youth groups in Louth and Horncastle, evening and after-school activities, and holiday clubs supporting self-care and independence.
This role involves working across both services with a supportive team, planning and facilitating high-quality sessions to support young people's targets and confidence. Our goal is to support all children and young people with disabilities or complex health needs, working together to ensure they are happy, healthy, safe, and thriving.
You will provide high-quality targeted support promoting self-confidence and independence, working as part of an integrated team. The Early Years Alliance will offer training and development opportunities to support your professional growth.
Key responsibilities include:
1. Planning and facilitating community, youth, and holiday sessions in local venues, ensuring activities are appropriate, safe, and promote confidence and independence.
2. Assessing, tracking, and monitoring children and young people's progress related to their EHCP outcomes and personal development using tools like Tapestry and Outcome Wheels.
3. Supporting parents and caregivers to access advice from the Local Offer and encouraging peer participation and mutual support networks.
Qualifications and experience required:
1. NVQ Level 3 in Early Years/Childcare, Youth/Playwork, or equivalent, or relevant experience working with children with disabilities or complex needs.
2. Understanding of the needs of disabled or complex health needs children and young people.
3. Effective communication skills, both verbal and written, with professionals and families.
4. Experience working within an integrated team with social care, early help, educational providers, and voluntary organizations.
