Mid Norfolk Mencap is looking for a dedicated Community Support Worker to join our Outreach team. In this full-time role, you will provide one-to-one support for adults with learning disabilities, helping them to thrive in their homes and local communities. What You'll Do As a Community Support Worker, you will: Provide tailored 1:1 support at home and in the community Help individuals build independence through support with daily living tasks such as shopping, cooking, managing money, and accessing health or leisure services Encourage social inclusion and support people to try new experiences Work alongside families, other professionals, and team members to ensure truly person-centred care Support individuals to make informed choices, while respecting their dignity, rights, and privacy Maintain accurate records and follow care and support plans Monitor wellbeing and report any changes to the management team Be a key contact for individuals you support, helping to coordinate their care and maintain updated support plans Who We're Looking For Wed love to hear from you if you are: Caring, patient, and passionate about supporting people to live their best lives Able to work flexibly and adapt to changing needs Skilled in communication and committed to maintaining professional standards A driver with access to your own vehicle this is essential Willing to complete all required training and take part in ongoing professional development A background in care or support work is desirable but not essential your attitude, values, and commitment matter most.
