This role exists to ensure that each individual we support receives person-centred care that meets their specific needs, promotes independence, and enhances their overall quality of life. Staff in this role play a key part in delivering daily support, safeguarding wellbeing, and helping individuals build life skills, confidence, and social connections.

What does this Support Worker job involve?
Hours of Work: 37.5 Hours Per Week - Shifts: 07.00 - 14.30 / 14:00 - 21:30
* Providing personalised support with daily living tasks such as cooking, cleaning, shopping, and personal care (where required)
* Supporting individuals to access the community – attending appointments, social activities, education, or leisure opportunities
* Encouraging independence and skill-building, helping people to achieve their personal goals
* Building positive relationships with the individuals we support, their families, and other professionals
* Supporting with emotional and behavioural needs, using person-centred approaches and following support plans
* Ensuring a safe, clean, and comfortable environment in line with health and safety standards
* Keeping accurate records and documentation, including daily notes, incident reports, and care updates
* Administering medication (after training, if required) and monitoring health and wellbeing
* Working as part of a team, attending handovers and contributing to care planning and reviews
