Clinical To participate in the provision of direct care by undertaking initial assessment and diagnosis of patients leading to endpoints of advice, treatment and/or referrals within the nurses scope of practice and WIC policies, guidance and procedures. To be professionally, legally responsible and accountable for the delivery of healthcare for patients presenting at the WIC and for all aspects of own work. To interpret and analyse clinical and non-clinical facts and use clinical reasoning based on evidence based knowledge to formulate individual diagnoses and treatment plans and utilise a wide range of treatment options to develop the most appropriate plan of healthcare. This may include: the supplying or prescribing of medicines discharge with follow up as required health education to encourage self-help and provide health Care advice as required.
direct referral to other health, social or community services per agreed pathways Undertake induction working towards agreed competencies including: physical assessment, history taking, diagnosis, treatment and referral or discharge and documentation including treatment, supply of medicines under WIC patient group directions. To appropriately identify and refer patients who present with clinical conditions that are beyond the scope of WIC care via agreed pathways To provide specialist advice where required to WIC colleagues where you have a recognised area of clinical expertise To use all equipment in a safe and appropriate manner at all times. To accurately analyse and interpret diagnostic test results done in the WIC To accurately and professionally document all clinical findings and outcomes utilising accepted documentation tools and within existing legal frameworks. Organisational To accept clinical responsibility for seeing users on a first come first served basis and to organise this effectively and efficiently with regard to clinical priorities.
To implement and adhere to Putnoe Medical Centre policies To attend meetings/briefings where required. Communication To communicate complex and sensitive information in an understandable form for users and carers who may be distressed or have difficulty coming to terms with a diagnosis and its implications. To communicate effectively with patients and carers to ensure understanding of the condition and appropriate health education and advice offered to maximise the benefit of the treatment plan. Takes cultural and social issues into consideration when planning care and treatment.
To assess capacity, gain valid informed consent and have the ability to work within a legal framework with patients who require an interpreter or advocate to consent to, and fully understand treatment. To be responsible for ensuring that appropriate methods are utilised to effectively communicate with users who have barriers to communication e.g. hearing impairment, cognitive difficulty, language barriers, etc. To represent Putnoe Medical Centre and the WIC (locally, regionally and nationally) in a positive and supportive manner and in line with their policies and procedures.
To communicate effectively and work collaboratively with medical, nursing and allied health colleagues to ensure delivery of a co-ordinated multidisciplinary service, e.g. referral letters, as well as appropriate communication with secondary care and the Child Protection Team.
