Role
Band 6 Registered Nurse working within the North West Rehabilitation Service as part of a dynamic multidisciplinary team. The role focuses on assessment, treatment, and intervention for older adults and disabled adults with complex needs, promoting independence, self‑management, admission avoidance, and safe hospital discharge. The nurse will independently manage a complex caseload and contribute specialist nursing expertise.
Duration, Location and Working Pattern
Fixed‑term, part‑time – maternity cover. Shift pattern: 26 hours over Monday to Sunday. Base location: Community Centre for Health.
Key Responsibilities
* Managing a clinical caseload independently, applying specialist nursing knowledge.
* Delivering person‑centred care focused on rehabilitation and maximising independence.
* Working within a multidisciplinary team to support assessment, treatment, and discharge planning.
* Supervising support workers and students.
* Providing professional nursing leadership, guidance and development within the team.
Qualifications and Experience
* Must be an RCN‑registered nurse.
* Experience in community nursing and multidisciplinary teamwork.
* Relevant background in neurology.
