We have an exciting role on offer within our Public Health Service as a Support and Engagement worker – Outreach team.
Are you passionate about supporting people with substance use issues and making a real difference to the lives of some of the most vulnerable members of our community? We are looking for a motivated and compassionate Substance Use Outreach & Engagement Practitioner to join our dedicated team in Middlesbrough.
This role provides flexible, trauma-informed support to people affected by substance use, many of whom face multiple challenges including housing issues, mental and physical ill health, offending behaviour, and social exclusion.
You will work creatively across a range of community settings, including outreach locations, service users’ homes, partner organisations, prisons, and courts, to reduce barriers to treatment and promote positive engagement. Using a strengths-based approach, you will help individuals access the services and opportunities they need to improve their quality of life and work towards recovery. You will carry out assessments, harm minimisation and brief interventions, develop and implement support and safety plans, undertake risk assessments, and work proactively with individuals who may present with behaviours that challenge.
The role requires confident multi-agency working, excellent communication skills, and a commitment to safeguarding, equality, and outstanding customer care. You will be part of a supportive team within Middlesbrough Council, with regular supervision, opportunities for professional development, and the chance to deliver meaningful, measurable outcomes for individuals and communities.
