Gloucestershire Health and Care are seeking an experienced Band 6 Later Life Nurse / Senior Mental Health Practitioner to join the Later Life Team at Avon House. This is a community-based role supporting older adults with severe and enduring mental health needs, ensuring recovery-focused care in the least restrictive environment.
The Role
Working as a senior member of the Later Life Team, you will:
* Undertake complex assessments and implement evidence-based interventions for service users with acute or long-term mental health needs
* Manage a caseload of approximately 25–30 service users, providing care coordination under the Care Programme Approach (CPA)
* Support service users to maintain independence and remain in the community whenever possible
* Deliver interventions in line with NICE guidelines and Trust policies
* Administer and monitor medication, including depot injections, and delegate appropriately
* Contribute to clinical governance, audits, and quality initiatives
* Liaise with multidisciplinary teams, social care, voluntary agencies, and families
* Supervise, mentor, and support junior staff, including assessment and induction where required
* Participate in team meetings, training sessions, and professional development activities
* You will regularly encounter challenging situations, including working with service users who are distressed, aggressive, or at risk, requiring strong clinical judgement and risk management skills.
Requirements
Essential:
* Registered Mental Health Nurse (RMN) with current NMC registration
* Post-qualification experience in mental health, including risk assessment and community care
* Knowledge of Mental Health Act legislation and evidence-based interventions
* Strong communication, documentation, and organisational skills
* Ability to work independently and as part of a multidisciplinary team
* Ability to travel independently across the locality
Desirable:
* Experience working with older adults in a community or later life setting
* Collaboration experience with voluntary sector or external agencies
* Familiarity with child protection or safeguarding processes where relevant
Pay Rates (Band 6 – NHSI Cap Rates – Paye/Umbrella)
Day: ÂŁ22.00 to ÂŁ26.00 per hour
