We are seeking 3 Female Full Time Support Workers for our Supported Living Service in Pontefract, WF8. Whilst it is not mandatory, preference will be given to applicants who hold full UK Driving Licence (to enable the people we support to engage in social and/or practical activities Shift patterns will include a mix of Long Days, Short Days, Sleep-In's and Waking Nights:-
1. Long day shift - 07.30-20.30
2. Long day into sleep shift - 07.30-22.00
3. Sleeping shift 22.00-07.30
4. Waking night shift 20.30-07.30
5. There are also half shifts which are:
6. 07.30-14.30
7. 07.30-15.00
8. 14.30-20.30
9. 15.00-20.30.

Who you’ll be supporting & more about the role:

The Service is a unique bungalow with 4 bedrooms. The tenants share the communal lounge and kitchen.

27. We provide 1:1 support for 4 tenants.
28. All tenants have an individual support plans, ensuring they are supported in a person-centred way and are promoted to make own choices.
29. Each person has their own individual care package of support.

31. All tenants choose individual activities and staff are matched to the tenant to promote inclusion and personal choice being respected and valued.
32. Working closely with families and significant others within the person we support circle of care.

34. We offer exceptional training and a professional induction into the service, ensuring you have the skills and knowledge to provide tailored support.
35. We will allocate you a “buddy” on your first day, to ensure you have an allocated person to assist in the transition into service.

38. We need highly reliable and motivated people with a passion to support out tenants in a person-centred manner.
39. We need you to be flexible to meet our needs and our service and our tenants.
40. We need you to able to have positive relationships with our families and the individuals highlighted in the circles of support.
41. We need you to commit to the shift patterns of 07.30 to 22.00 including a sleeping night which is 07:30 to 22.00 finishing at 07.30 the following day, and a waking night 20.30 to 07.30.
42. We need you to be committed to complete all the training and additional training required within your role.
43. We need you to be kind compassionate and be fun and think “out of the box”. Ensuring all support is delivered to ensure our tenants have the “Best life Possible”.
44. We need you to be friendly towards the people we support but remain within professional boundaries.
45. We need you to always communicate and listen to us and respect what we have to say and become a valued team player.
46. We need you to fully document all interactions and be committed to supporting all tenants and meet their care needs, to ensure all documents remain compliant.
