Clinical Learning and Physical Disabilities Nurse Responsibilities Healthcare Support: Assess and monitor the health needs of individuals with learning disabilities. This includes implementing care plans tailored to each individuals needs. Promoting Independence: working to empower individuals to help them live as independently as possible. Knowledge: Good understanding and knowledge of DOLs, MCA and Behaviour Management. Collaboration : collaborate with multidisciplinary team to provide comprehensive care, supporting families and caregivers by offering guidance, coaching and resources. Monitoring and Evaluation : regularly evaluate the effectiveness of care plans and make necessary adjustments based on the progress and changing needs of the individuals they support. Health Education : educate clients and carers about the clients own health and well-being, including hygiene, nutrition and managing medical conditions, adapting teaching methods to suit the cognitive abilities of each individual. Advocacy: Act as advocates for client families, ensuing that their rights are protected and that they have access to necessary services and support, including facilitating communication and making reasonable adjustment to care. Knowledge/Skills: Excellent communication and writing skills. Ability to travel across Newcastle and Gateshead Areas. Able to work on own initiative without direct supervision. Ability to maintain professional boundaries/ diplomacy whilst working in intense conditions. Able to work well within a team. Able to maintain confidentiality. Able to recognise own limitations and scope of practice. Knowledge of and willingness to use digital systems. Ability to stay calm in distressing and stressful situations.
