Overview

We are looking to recruit an Advanced Mental Health Practitioner to join our specialist Personality Disorder Community Service (PDCS). The role supports care planning and implementation in treatment pathways, with a strong focus on providing excellent clinical care within the multidisciplinary team (MDT). The role also includes support to the crisis pathway within the service.

PDCS is a countywide specialist outpatient service offering NICE-recommended treatments to adults aged 17–65 with personality disorder. Treatment options include Mentalisation Based Therapy (MBT), Dialectical Behavioural Therapy (DBT), an Occupational Therapy Pathway and an engagement and formulation pathway informed by a Structured Clinical Management model. The PDCS MDT emphasises reflection and risk management within a wrap-around approach.

Travel between bases in Peterborough, Cambridge and Huntingdon is part of the role, with some hybrid working.

Main duties of the job

As an Advanced Mental Health Practitioner in PDCS, you will work within a multidisciplinary team to provide treatment and system-wide support for adults with complex needs related to personality disorder, including those experiencing longstanding risk and crisis periods.

* The main focus is to support the coordination and implementation of the Level 4 Treatment Pathway.
* Provide support to the duty pathway as required by the service.
* Support system-wide management of PDCS service users, working with other agencies within and outside CPFT.

Travel and driving requirements: You will be required to travel independently around the county meeting strict time deadlines. A full UK driving licence and access to a vehicle are required. Please confirm you meet these criteria in your application.

Job responsibilities

* Deliver ongoing specialist assessments of adult service users referred to and under the care of the Personality Disorder Pathway.
* Develop formulations of presenting problems/information from assessments, individually and as part of the team.
* Develop and implement treatment plans and therapeutic interventions for a client’s presentation, normally within Mentalisation-based therapies, with other evidence-based approaches as appropriate.
* Oversee lower-banded staff developing formulations and delivering care, as required.
* Implement a range of interventions for individuals, carers, families and groups.
* Provide specialist professional advice, guidance and consultation to other professionals contributing to formulation, diagnosis and treatment plans.
* Undertake risk assessment and risk management in line with Trust and inter-agency policies, including work with vulnerable adults.
* Act as lead clinician for assessments and advising clients and other professionals under Safeguarding of Vulnerable Adults guidelines.
* Communicate sensitively with service users, family/carers and others about assessments, formulations and treatment plans.
* Maintain appropriate records of assessment and treatment in line with professional guidelines and Trust policy.


Person Specification

Education/Qualifications

* Essential: Evidence of relevant post-registration training; Higher Level Qualification relevant to the role (Non-Medical Prescribing, Postgraduate therapy) or equivalent experience and intention to complete.
* Desirable: Non-Medical Prescribing Qualification; Post-registration training in risk management.

Experience

* Essential: Experience as a Senior Practitioner with clients with a diagnosis of Personality Disorder; experience in a Multi-disciplinary Community Mental Health Team; minimum 5 years post-registration; experience providing clinical leadership and supervision to junior staff.
* Desirable: Experience of working within a Crisis pathway; experience with wider stakeholder groups (Care Pathway Teams, independent sector agencies).

Skills & Abilities

* Essential: Ability to develop therapeutic relationships with difficult-to-engage clients; good understanding of Recovery Principles.
* Desirable: Experience in audit; understanding of case management in teams.

Knowledge & Understanding

* Essential: Knowledge of the difference between acute and chronic risk in personality disorder.

Other

* Essential: Ability to work with and tolerate high levels of distress in others; ability to travel to multiple locations within the Trust as required.
