Overview
6 - 8 sessions (25 - 33.5 hours). We are recruiting a part-time Advanced Nurse Practitioner to join Amersham Vale Practice in Deptford, Lewisham. We are a 15,800-patient training practice based in the Waldron Health Centre. Deptford is a diverse area with a mix of development and deprivation; we are opposite New Cross station and near Deptford DLR and Deptford station. We value kindness, approachability, friendliness and compassion, alongside high standards of clinical care. Our working environment is supportive and collaborative, with regular tea breaks. We are a training practice with a diverse clinical team including Advanced Nurse Practitioners and a Clinical Pharmacist, and we teach medical students, physician associates and GP registrars. We work as a team and value everyone’s contributions.
We run a dedicated clinic for the homeless, for asylum seekers and a service for drugs and alcohol addiction, with close links to Goldsmiths University. We are committed to healthcare for all and to ensure everyone benefits we reach out to patients who society disadvantages. We are a Gold standard practice for the LGBTQ+ community.
Responsibilities
* You will be part of the ANP team which includes 2 other ANPs and one paramedic.
* You will support duty sessions with triaging of patients and will have your own designated clinics to call patients or see them face to face to address more complex health needs; you will also conduct regular home visits.
* You should be able to see all patients including children, adults and those with mental health challenges.
About the role
* We are diversifying how we work where you will triage and treat during sessions, with on-site clinics and home visits for complex cases.
* Delivery of care in line with NSF, NICE guidelines and evidence-based care.
* Provide care within a team of GPs, ANPs, a clinical pharmacist, practice nurses and HCAs.
Key duties as part of the ANP role
* Coordinate planning and delivery of care with patient and carer involvement.
* Prescribe and review medications within scope of practice and current guidelines.
* Diagnose and manage acute and chronic conditions; refer as appropriate.
* Request and interpret diagnostic tests (e.g., x-ray, bloods, scans).
* Assess, diagnose and treat in practice or patient homes; refer to appropriate services as needed.
* Refer patients to other team members or secondary care as necessary.
Qualifications and experience
* Essential – Registered first level nurse; MSc or equivalent; independent prescribing qualification; able to see all patients including children and those with mental health challenges; 2 years experience in triage; non-medical prescriber.
* Desirable – Experience in primary care/general practice; case management; nurse-led triage; mentoring/teaching qualifications.

* Management of long-term conditions and patients with complex needs; clinical governance knowledge.
* Ability to lead, teach and mentor in a clinical setting; strong organisational and time-management skills; flexible and adaptable; team player.
Other requirements
* Disclosure and Barring Service (DBS) check as per Rehabilitation of Offenders Act requirements.
* Current UK professional registration.
