Adults LDAT and Transitions Social Care Lead

Ref: 001111

Contract: Permanent

Salary: £51,356 - £52,495 per annum (Grade 10)

Hours: 36.66 per week

Based: Spindles Office, George Street, Oldham, OL1 1HD

Role Overview

The Team Manager to lead the Adult Learning Disability, Autism, and Transitions Team, delivering person‑centred support for adults with complex needs. The role involves operational leadership, multi‑agency collaboration, and continuous service development to ensure smooth, coordinated care pathways.


Key Responsibilities

* Lead and manage a multidisciplinary team, ensuring effective supervision, performance, and professional development.
* Coordinate complex transition planning for young people entering adult services, ensuring timely assessments and person‑centred outcomes.
* Develop and maintain strong partnerships with health, education, social care, housing, and voluntary sector agencies.
* Monitor service delivery to ensure legal and regulatory compliance, safeguarding, and best practice standards are met.
* Use data and feedback to drive continuous improvement, service planning, and resource allocation.


Key Requirements

* Proven experience in managing or leading teams within adult social care, learning disabilities, autism, transition or Adult community services.
* Strong knowledge of relevant legislation and frameworks, including the Care Act 2014, Mental Capacity Act, and transition pathways from children's to adult services.
* Excellent communication, leadership, and partnership‑building skills to work effectively across multidisciplinary and multi‑agency settings.
