Service Coordinator
Location: Hull, with travel around East Yorkshire
Salary: £25,155 per annum, pro rata
Hours: 21 per week
Contract Type: Fixed term until end February 2026
A Full UK driving licence and access to vehicle required for this role.

Overall purpose of the role

You will be responsible for making settling in calls to patients that have recently been discharged from hospital to check on their patient experience and interactions with health and care services). Insights captured through settling-in calls will be fed-back to the relevant system partners to support continuous improvement of health and care services.

Whilst making the settling in calls to patients, you will make sure that individuals are safe and well at home and their needs are being met, and if needs are identified the coordinator will refer and signpost to other statutory or non-statutory services to make sure the individual receives the right level of care and support.

You will be responsible for liaising with hospital staff and other medical professionals, to receive patient referrals. Thorough records will be required to evaluate and analyse the outcomes of the services. You will assign incoming referrals to a Service Assistant (or equivalent) and ensure the team works together seamlessly.

What will a day in the life of a Service Coordinator look like? You'll be: Responsible for building strong positive relationships and liaising with hospital staff and other medical professionals in the community, including GP surgeries, to receive patient referrals.
Managing a team of staff and volunteers.
Evaluating and analysing the outcomes of the services, assigning incoming referrals to support workers.
Monitoring the service delivery, ensuring service health and safety and evaluating progress.
Overseeing record-keeping and recruiting.

What does it take to be a successful Service Coordinator? You'll need: IT literacy.
Understanding of how to improve service quality for the benefit of users.
Full UK driving licence holder and access to vehicle.
Willingness to work flexible hours.
Ability to deal with queries in a diplomatic, professional and confidential manner.
