Overview
An excellent opportunity for a band 6 Anticoagulation Nurse to join the Anticoagulation Team within the Pathology Department. You will analyse and interpret technical investigations of clinical samples to dose patients according to current anticoagulation guidelines. You will contribute to high-quality, clinically effective care and influence all aspects of patient management within the specialty.
Responsibilities
* Clinical assessment of patients – indications for therapy, therapeutic ranges, and duration of therapy.
* Undertake Anticoagulation clinics in hospital, community, and patients’ homes.
* Monitor and dose patients in anticoagulation clinics and in patients’ homes; refer to other clinicians or services as required.
* Educate newly referred patients and their carers on anticoagulation management and monitoring.
* Perform diagnostic testing using Near Patient Testing devices (e.g., Coaguchek) to obtain INR results.
* Prescribe anticoagulation therapy in accordance with local guidelines after assessment of INR or creatinine clearance.
* Initiate oral anticoagulation for differing conditions and manage bridging with LMWH when appropriate.
* Offer advice and information on anticoagulation management to clinicians, patients, and carers.
* Perform phlebotomy as required to obtain venous blood samples.
* Influence all aspects of patient care within the specialty and promote delivery of high-quality, clinically effective care; prioritise and utilise research in daily practice.
Qualifications and Requirements
* Registered Nurse of the appropriate part on the NMC Register.
* NMP Qualification.
* Proficient in Phlebotomy.
* Previous experience within an anticoagulation setting.
* Excellent communication and interpersonal skills.
* Driver prepared to travel throughout the borough of Walsall.
