Occupational Therapist

Be an active member of the integrated multi-disciplinary team for inpatients at Priscilla Bacon Lodge with palliative and end of life care needs.

Be responsible for a defined caseload, using evidence based/person-centred principles to assess, plan, implement and evaluate occupational therapy interventions.

To work as part of a multidisciplinary team within a quality framework to ensure that the highest standards of care are met.

To support and deliver innovation within discharge practice and pathways.

To work with system partners including the voluntary sector to ensure that patients experience a seamless transfer of care both into and onward from Priscilla Bacon Lodge.

Be responsible for the delivery and co-ordination of care to a named group of patients.


Main duties of the job

To work with the inpatients at Priscilla Bacon Lodge to assess their needs for equipment and discharge planning. To support what is important to each patient.

To support each patient who is being discharged with where they are to be discharged to, and support the patient and family with the transition of care from one clinical area to the desired destination.

to ensure all decisions are made with the patient and where the patient wishes with the family as well

To maintain own professional development and undertake opportunities for learning and developing transferrable skills to use in other clinical areas

Job responsibilities

To be professionally and legally responsible and accountable for all aspects of own work.

To undertake assessments of patients and deliver a formulated individual management plan using clinical reasoning skills and a range of treatment skills

To communicate effectively with patients/families/members of the MDT

To support therapy assistants and assist and support senior staff

to be responsible for equipment used, for ordering equipment as required and ensure competence in equipment use

To apply evidence based practice and participate in audit as requited

To be aware of challenging and complex situations and seek advice and support as required

To maintain comprehensive and contemporaneous patient records

To maintain professional registration and adhere to own Code of Ethics as well as Trust and Professional standards

To ensure professional and personal development, and participation in supervision

to respect individuals, their rights, values, culture and religious diversity and ensure care is sensitive to needs

To ensure consideration is given to infection control, Health & Safety, and safeguarding needs of patients


Person Specification


Qulaifications

* BSc/MSc in Occupational Therapy
* HCPC registration


Experince

* Experience of working with people who have palliative and end of life care needs
* Broad range of core clinical experience post registration or pre-registration
* Can demonstrate ability to problem solve in a clinical setting
* Demonstrates ability to be an effective and adaptable member of a team to meet patient and service needs
* Post registration experience in a relevant clinical setting
* Relevant paid or voluntary work
* Advice and support to students and support staff


Skills and Abilities

* Demonstrates ability to apply skills across various clinical setting/applications
* Demonstrates understanding of clinical audit, evidence based practice, frequent changing needs of patients, commitment to CPD
* Shows well developed problem solving skills
* Able to communicate effectively in written and verbal English
* Knowledge of palliative and end of life care legislation and risk management
* Experience of SystmOne
* Have basic knowledge of sensory processing
* Knowledge of breakaway/de-escalation techniques
* Experience of LAS or system 1 or equivalent IT
* Ability to reflect on and critically appraise others and own performance.
* Adaptable to change
* Attention to detail and acts with integrity
* Able to identify with the Trust's commitment to safeguarding and promoting the welfare of children and young people/vulnerable adults.


Other

* Has a current full driving licence and can make own travel arrangements
* Awareness of hazards of lone working
