1 x Band 6 Specialist Occupational Therapist for North or Central Aberdeenshire

Aberdeenshire’s Occupational Therapy Service is delighted to offer a new Band 6 post for an experienced and dynamic Occupational Therapist to join our community rehab teams and help shape and deliver the Discharge to Assess (D2A) model across the region. We are seeking motivated, enthusiastic, and forward-thinking professionals to join our teams based in North or Central Aberdeenshire. The role will be pivotal in supporting the early discharge of patients from hospital, and enabling safe and effective rehab and enablement at home.
Responsibilities
* Provide a priority response to facilitate timely hospital discharge.
* Provide a priority response for patients referred to our virtual community wards, enabling them to remain at home wherever possible.
* Work within a multi-disciplinary and multi-agency framework to ensure seamless care in the community.
* Manage a diverse caseload with a wide range of conditions and complex needs.
* Supervise and allocate work to OT support staff and contribute to student training.
* Play an active role in service development within your locality and the wider Health & Social Care Partnership.

Requirements
