Responsibilities:
4. Assess individuals in hospitals for care packages or care home placements under the Care Act.
5. Conduct Mental Capacity Assessments for care and support.
6. Collaborate with multidisciplinary teams to create care plans.
7. Engage in carers conversations.
What we are looking for in a Social Worker:
8. Social Work England registration
9. Eligible to work in the UK
10. Hold a full UK licence
11. Post-qualified experience
12. Degree level or equivalent in Social Work
