Our Health in Justice service line is now expanding into non-Custodial healthcare, a person-centred approach delivered via a solid and consolidated infrastructure aimed to assess individuals' mental health and neurodiversity requirements, to redirect them to the most appropriate channel. We have an exciting opportunity for a Clinical Liaison & Diversion Practitioner
to join our team in Humberside.

Salary

This position is full time and you will receive a salary of £42,094 to £47,466. depending on experience

Candidates are required to have a Legal right to work in the UK due to the nature of the role, offers of employment will be subject to additional vetting and security checks.

As a Clinical Liaison & Diversion Practitioner you will:

* work autonomously in court and police custody suites, to identify and respond to requests of screening and assessment of people with identified health needs in contact with the criminal justice system
* assess for several identified health needs, create a clear individualised support plan
* prepare accurate written reports such as a court report following assessment in line with quality standard
* be responsible for the development of a support plan, based on the identified needs from assessment to ensure that we are contributing to the reducing reoffending strategy by helping to address any outstanding health and social needs of the service user
* work closely with partner agencies to ensure timeliness of information sharing with key stakeholders to ensure that pertinent and relevant information is given to inform decision making within the court and custody arena
* provide supervision and support to junior team members, providing clinical advice and support as required to support the ongoing assessment and support for our service users within our care.

You will support the Head of service and Clinical lead in the service development, governance and audit schedule to ensure continuous improvement and high-quality care is maintained

Qualifications, experience and skills required:

* Qualified Registered Mental health Nurse, Mental Health Social Worker or Occupational Therapist with current registration with the relevant professional body
* Substantial evidence of continuing professional development and post registration training.
* Experience of working with patients, face to face, who have complex mental health conditions
* Awareness of psychosocial interventions available to service users
* Awareness of substance misuse services and support available

We will treat individuals more holistically, focusing on the person and early intervention strategies to reduce risks and improve outcomes.
