Rotational Occupational Therapist Band 5
An exciting opportunity has arisen for a Band 5 Rotational Occupational Therapist to join the East Coast Community Healthcare Rotation, covering Neurology, Primary Care Network (community), and Wheelchair services.
The Integrated Community and Neurology Team - The team supports patients with long-term neurological conditions across the Great Yarmouth and Waveney area. It consists of specialist nurses, physiotherapists, Occupational Therapists, and rehabilitation assistant practitioners. Assessment and intervention are completed in clinic settings as well as in patients' homes and care homes. We provide timely assessments and treatments to facilitate and improve patients' quality of life and maximize independence. You will be responsible for a designated caseload, supporting patients throughout their journey from initial diagnosis to management and palliative care.
Primary Care Homes - The team supports patients in the community with complex care needs requiring specialist occupational therapy to improve their occupational performance. You will work as part of multidisciplinary teams including nurses, occupational therapists, paramedics, rehabilitation support workers, assistants, practitioners, pharmacy support, healthcare assistants, and community matrons.
Main duties of the job
Wheelchair Services - The service aims to aid mobility, independence, wellbeing, and quality of life for individuals with long-term disabilities or medical conditions affecting walking ability for six months or more. Clinicians provide thorough assessments to determine the most beneficial wheelchair within NHS guidelines and eligibility criteria. Assessments include postural assessments, pressure relief, and adjustments of NHS wheelchairs to meet specific needs, with reassessment when needs change.
The successful applicant will demonstrate high clinical expertise, excellent communication, and organizational skills.

Job responsibilities
CLINICAL:
To be professionally and legally responsible for all aspects of your work, managing patients' care to high standards. To interpret clinical and non-clinical facts, form diagnoses and prognoses, and develop management strategies.
To undertake comprehensive assessments, formulate individualized treatment plans, and use clinical reasoning to provide a specialized care program.
To accept clinical responsibility for a caseload, organize effectively, and prioritize appropriately.
To develop assessment and manual treatment skills, safely use therapy equipment, and support training from senior therapists.
To recommend and prescribe equipment and minor adaptations, ensuring safe use and instructing patients and carers accordingly.
To assess, measure, and communicate recommendations for adaptations to providers with support from senior staff.
To be aware of psychological risk factors and implications.
To provide specialist advice, teaching, and instruction to promote understanding and ensure consistent patient care.
To supervise assistants and support staff when appropriate, and supervise students during placements.
To communicate effectively with patients, carers, and team members, working within legal frameworks for capacity and consent.
PROFESSIONAL:
To maintain ongoing CPD, participate in training, and keep accurate documentation per legal and organizational standards. To adhere to professional guidelines and codes of conduct. To have knowledge of relevant equipment and resources, and evaluate work through evidence-based practices and audits.
To demonstrate understanding of Clinical Governance and Risk Management.
ORGANISATIONAL:
To manage a designated area of work efficiently, prioritize tasks, and contribute to data collection and service development. To ensure safety policies are followed and organizational procedures are adhered to. To undertake additional duties as assigned by senior staff.

Experience
* Evidence of clinical placements or previous relevant experience as a Band 5 Occupational Therapist
Qualifications
* Diploma/Degree in Occupational Therapy
* HCPC registration
* Evidence of CPD and Preceptorship

* Knowledge of occupational therapy relevant to the area
