CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

Would you like to work with a creative and enthusiastic team who will support and help you develop your clinical, managerial, personal and professional skills?

We specialise in general rehabilitation & offer a 17-day pathway for patients with a variety of comorbidities. The post holder will be required to complete assessments, interventions and rehabilitation programmes to aid the recovery of patients admitted to St Peters ward.

The ideal candidate will be a highly motivated individual, who is passionate about helping patients to improve their quality of life & helping staff to develop in their roles and are keen to learn. You will be working with a dedicated multidisciplinary team. The successful candidate will be able to work autonomously, have excellent communication & time management skills with the ability to problem solve and prioritise.

In this role you will be required to supervise a band 5 occupational therapist and students within the team. In addition to assisting with the team with the patient caseload, service development and quality improvement projects you will have the opportunity to deputise for the Band 7 OT.


Main duties of the job

* To be responsible for the provision of Occupational Therapy in our Non weight bearing beds and Rehabilitation Beds ensuring that services support the discharge of patients by providing specialist assessment and discharge planning.
* To work as part of a team including a physiotherapist, rehabilitation assistant, Adult Care Services and care home staff to facilitate timely assessment and timely, co-ordinated discharge planning at Forest Care Village
* To work as part of a multidisciplinary team in the rehabilitation units to facilitate timely assessment and timely, co-ordinated discharge planning as required
* To hold responsibility for own caseload, working with and without direct supervision.
* Provide support for the clinical supervision student Occupational Therapists and give direction to members of support staff.
* To work flexibly, being willing to work on other Units if / when required.

Person Specification


Qualifications

* Diploma/Degree in Occupational Therapy.
* Registration with the Health and Care Professions Council
* Relevant Post graduate training.


Education/Qualification

* Diploma/Degree in Occupational Therapy

* Relevant Post graduate training.


Experience

* Knowledge of current best practice in OT
* Ability to work single-handedly with individuals and groups
* Understanding of professional ethics and their application in practice.
* Understanding of team dynamics
* Knowledge of health legislation and current practice, risk assessment and management.
* Effective written and oral communication skills
* Word processing and internet skills
* Knowledge of the principals of clinical governance
* Ability to reflect and critically appraise own performance.
* Health, safety and risk awareness
* Commitment to client-centred, non-discriminatory practice
* Commitment to lifelong learning


Skills and Knowledge

* Able to be organized, prioritise and manage own time.
* Able to accept advice and feedback and to act upon this
* Able to work both independently, and with direct and indirect supervision.
* Interest in working in community settings.
* Willingness to work flexibly, at a variety of locations.
* Evidence of active CPD portfolio
* Car Driver, Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
