We are looking to recruit a skilled and compassionate Social Worker to join our East Locality Assessment & Care Planning Team in Falkirk, supporting adults and their carers to live safely, independently, and with dignity.
The Role
* Complete strengths-based assessments and develop outcome-focused care and support plans
* Support adults and their carers to maintain independence and wellbeing
* Work closely with health colleagues and partner agencies in a multi-disciplinary setting
* Manage a varied caseload, including complex needs
* Contribute to adult protection work where required
* Maintain accurate and timely case recording and reports
What We're Looking For
* Degree or equivalent qualification in Social Work
* SSSC registration (or eligibility)
* Experience in adult social work (community-based experience desirable)
* Strong communication, assessment, and risk management skills
* A person-centred, strengths-based approach to practice
