Job Title and Service
Team Lead Occupational Therapists – North West and North East Localities, Community Rehabilitation: Prevention of Admission, Discharge to Assess and Reablement.
Job Summary
The role involves leading and managing a team of Occupational Therapists and Assistant Practitioners across the Discharge to Assess, Prevention of Admission and Reablement work streams. You will support patients to return home from hospital and enable them to remain at home rather than being readmitted.
Core Responsibilities
* Lead and manage the team of Occupational Therapists and Assistant Practitioners across the three work streams.
* Provide multidisciplinary leadership for NHS and Local Authority Occupational Therapists.
* Collaborate daily with Physiotherapists, Assistant Practitioners and Community Therapy assistants as part of an integrated community team.
* Support patients to return home from hospital in a timely manner and prevent unnecessary admissions.
* Maintain high standards of discharge planning and clinical service delivery.
* Be based in one of the four Community bases across the city, based on service needs.
Essential Requirements
* Excellent organisational skills and a clear, kind, effective communicator.
* Previous experience working in a rehabilitation or community setting with highly developed discharge planning skills.
* Regulated work with vulnerable children and/or protected adults under the Protection of Vulnerable Groups (Scotland) Act 2007.
* Commitment to join or renew a PVG Scheme check before formal offer.
* Eligibility to be employed by either City of Edinburgh Council or NHS Lothian.
