To lead on the case management of clients and be part of a team that supports them in keeping them safe and helping them achieve their goals.
To provide people with homes and specialist support so they feel more valued and secure, and ready to take the next steps.
Main Responsibilities
1. To assess the needs of each client, helping to develop their individual support plan to achieve their goals.
2. To produce and regularly review comprehensive client-based risk assessments.
3. To manage a caseload of clients on a one-to-one basis, providing personalised, intensive support via assertive outreach to long-term rough sleepers in the local area.
4. To engage with local authorities, health providers, and other agencies, creating and maintaining constructive working relationships to maximise the opportunities available to clients.
5. To work with individuals to create clear housing plans, and to support individuals to access support services around health and wellbeing, money, benefits, and housing.
6. To engage and support rough sleepers on the street working alongside local health care teams.
7. To help keep clients safe by reducing their contact with police and emergency services.
8. To identify barriers to accessing and maintaining safe tenancies and prevent eviction.
9. To attend community meetings and presentations with managers to support additional funding.
10. To support clients in trauma-informed and psychologically informed ways.
11. To adhere to safeguarding responsibilities, following our safeguarding policy and procedure.
12. To help maintain health and safety, ensuring clients, visitors, and buildings are safe in accordance with Health and Safety regulations, policy, and procedures.
13. To provide basic first aid assistance until help arrives (full training is provided).
14. To work confidently and efficiently when lone working.
15. To maintain accurate records on our client management system and ensure these are stored in line with GDPR.
16. To maintain confidentiality.
17. To represent the organisation in a professional manner.
18. To carry out any other reasonable duties required in the interest of the organisation.
Role Requirements
1. This role will require an Enhanced with Barred List(s) disclosure and barring service check, which will be completed on appointment.
2. A full driver’s licence and access to a vehicle are required.
3. The ability to work flexibly across several sites is necessary.
4. Work on a one-to-one basis with clients is essential.
5. Night and/or weekend work is required.
6. Lone working capability is necessary.
Essential Criteria
1. Strong communication skills, both verbal and written.
2. Clear verbal and written English.
3. Good keyboard skills for data inputting.
4. Proficiency in Microsoft Outlook, Excel, and Word.
5. Confident and assertive manner.
6. Ability to self-motivate for independent work.
7. Effective team working skills.
8. Ability to respond calmly to crises.
9. Prompt and effective reaction to challenging situations.
10. Genuine concern for homelessness and related issues.
Desirable Criteria
1. Experience managing a caseload of clients.
2. Understanding of the complex support needs of people experiencing homelessness or with complex needs.
3. Knowledge of voluntary and statutory agencies, housing regulations, and the benefits system.
4. Understanding of risk assessment and person-centred, outcomes-based delivery.
