Job Description

We’re seeking an experienced professional to join our team in a permanent capacity, where your expertise and commitment will help shape lives and strengthen communities.
As a permanent, full‑time Social Worker you will work across West Lancashire as part of our Adult Services Older Adults and Physical Disability Community Teams.
As part of your new role you will be expected to undertake strength‑based, outcome‑focused assessments and support planning, ensuring that residents receive the right level of support. You will play a vital role in supporting people to access local services, maintain independence, promote health and wellbeing, and live independently. You will work alongside other agencies, professionals, and informal carers to provide a holistic service.

Qualifications
* Social Work applicants must be registered with Social Work England (SWE) to be appointed as a Social Worker or have applied to register with SWE.
* Applicants will be required to apply to the Disclosure and Barring Service (DBS) for a 'disclosure'.
* Applicants must provide a car for use in connection with the duties of this post and they must be insured for business use. However, we may consider candidates if they cannot drive because of a disability.
