Main duties of the job
Our Support Workers are the unsung heroes, providing crucial assistance to customers in the comfort of their homes post-hospital discharge. Collaborating with dedicated occupational therapists, we co-develop personalised reablement plans to help customers achieve their goals at home. The team operates on a rotating schedule, available between 7 am to 10 pm, 365 days a year, ensuring continuous support.
As a Senior Support Worker, you will monitor, support, and supervise our Support Workers, ensuring the delivery of high-quality services to our customers.
We welcome candidates with either an NVQ Level 3 or equivalent substantial experience in a related field, and those interested in pursuing relevant qualifications.

Job responsibilities
Specific duties and responsibilities
* Work closely with occupational therapy staff to create person-centered, CQC-compliant reablement support plans.
* Attend initial customer visits to introduce the service and support understanding of the reablement approach.
* Undertake risk assessments, including environmental, medication, and mobility risks, and manage these in collaboration with customers and support workers.
* Collaborate with partners such as health teams, care homes, agencies, and hospitals, ensuring safe transfer of cases and relevant information.
* Assist in providing safe care aligned with regulations and standards, including leading out-of-hours coverage and understanding CQC regulations.
* Maintain thorough and well-organized paperwork required by CQC, such as medication records, ensuring inspection readiness.
* Facilitate effective communication between professionals, customers, and families to promote partnership and reduce duplication.
* Provide customers with information about the service, including expectations, contact details, and raising concerns.
* Accurately record all contacts and maintain confidentiality, documenting actions, events, and decisions.
* Monitor progress toward reablement goals, providing feedback to occupational therapists and team members.
* Ensure smooth operation of the service under the guidance of the Team Leader/Registered Manager, with good organizational skills and flexibility.
* Report changes in support plans promptly, liaise with the coordinator for rostering, and support reablement workers to deliver services as planned.
* Manage emergency issues and out-of-hours situations, making decisions with authority.
* Participate in customer transfers to other support services as needed.
* Engage in supervision, training, and staff development activities, including induction and mentoring of new staff.
* Monitor service delivery quality through observations, supervision, spot checks, and audits.
* Maintain awareness of safeguarding policies, making referrals and participating in investigations to protect customers.
* Represent the Council professionally at all times, responding appropriately in civil emergencies, and challenging practice for service improvements.

Salary: £15.06 to £16.08 per hour (plus 10% unsocial hours pay for qualifying shifts).
