The Hospital Discharge Teams work with individuals to promote a person-centred approach to discharge planning. The Hospital Discharge Team West primarily covers Victoria Hospital in Kirkcaldy and Queen Margaret Hospital in Dunfermline. The East Team covers the community hospitals in East and Northeast Fife and Fife residents who are in Ninewells and Perth Royal Infirmary Hospitals.
The successful candidates will focus primarily on working with individuals in mental health wards in Queen Margaret, Whytemans Brae and Stratheden Hospitals.
You will carry out personal outcome focused assessments as part of a multi-disciplinary team and work with individuals to promote their rights and to achieve better outcomes. You will support people to move on from hospital to more appropriate settings.
The Person
You will be required to demonstrate knowledge and skills in working within a statutory adults setting. You will have a sound understanding of theory, policy, and legislation in relation to adults across the lifespan and be confident in the use of these within assessments.
You must be able to work within a multi-disciplinary team and display excellent communications skills, both verbal and written. You must be able to demonstrate an ability to engage with adults and their families and display a clear understanding of their needs.
An understanding and experience of working in a secondary setting is desirable as well as knowledge and experience of working with those who have a mental health problems.
Good skills in caseload and performance management are crucial to provide a high-quality service whilst meeting Scottish Government standards on delayed discharge.

You will be required to travel throughout Fife.
You will hold an Honours Degree in Social Work or equivalent.
You will be an accredited Mental Health Officer.
You will be registered with the SSSC and maintain the CPD requirements for this.
