Our 24-bedded unit provides rehabilitation for patients with varying needs, following medical or surgical treatment, or directly from their homes. We also offer palliative and end-of-life care.

The role involves rotating between day and night shifts. Our focus is on continually improving the quality of care and access, helping patients transition seamlessly between services.

Main duties of the job
1. Implement care based on individualised care plans and provide high standards of direct patient care.
2. Collaborate effectively with team members and establish good relationships with patients to support their needs.
3. Involve patients in decision-making, communicate relevant information to patients, families, and carers.
4. Document all care interventions accurately and participate in induction and mentoring of new staff.
5. Use ward resources effectively, maintain equipment, and act as a role model for junior staff and students.
6. Engage in problem-solving related to your work and organisation, and ensure patients and carers are informed about health needs.
7. Promote and participate in clinical supervision.

Job responsibilities
1. Carry out instructions from registered nurses or associates, ensuring safe and effective care.
2. Perform duties within the Clinical Support Worker Core Task List, within assessed competence.
3. Report concerns or changes in patient condition to registered staff.
4. Support emotional well-being through positive interactions and participation in activities.
5. Accurately document care electronically, maintaining confidentiality.
6. Respond appropriately to emergencies, knowing role actions.
7. Adhere to trust policies, procedures, and guidelines.
8. Assist with patient admission, preparation for treatments, investigations, and procedures.
9. Support relatives and assist with discharge procedures under supervision.
Person Specification
Qualifications (Essential):
* GCSE Grade 5 in English and Mathematics or equivalent
* Care Certificate or equivalent healthcare experience
Desirable:
* City and Guilds NVQ Level 3 in care/health and social care
Experience (Essential):
* Some experience in a nursing or care assistant role
