1. Support the start-up and stabilisation of new complex care packages across the UK
2. Assist Care Managers and clinical teams with safe and efficient hospital discharges
3. Step in where needed to provide continuity, stability, and reassurance to clients and their families
4. Mentor new Support Workers, role-modelling best practice and safe, effective care
5. Deliver personal care and daily living support tailored to each client
6. Establish routines, organise essential supplies, and manage prescriptions or consumables when required
