Responsibilities
* Conduct assessments to identify the needs of individuals with mental health challenges
* Develop and coordinate care plans and support packages to promote independence and recovery
* Support transitions between hospital, community, and other care settings
* Collaborate with healthcare professionals, families, and community services
* Advocate for service users to ensure their voices are heard and needs are met
* Maintain accurate records and comply with statutory guidelines
* Provide support in both community and hospital environments
Requirements
* Social Work qualification and registration with SWE
* Experience in mental health services (community, inpatient, or hospital discharge)
* Strong communication, assessment, and organizational skills
* Commitment to enhancing the well-being and recovery of individuals with mental health challenges
* Fluency in Welsh (desirable, depending on location)
