Job summary
South Warwickshire University Foundation Trust is looking to recruit experienced Clinical Practitioners to work alongside other members of the Urgent Community Response team in supporting admission prevention for patients living in the South Warwickshire area.
We can offer a flexible approach to working with an excellent induction plan, ongoing training and development within the role.
The successful candidate will be working with highly skilled members of the team who support each other to provide patients with individualised care based on their needs.
This team approach to patient care has proven to be very effective in ensuring patients remain at home.
Candidate will need to have both the Health Needs Assessment and Independent Prescribing qualification which are required for the role.

Main duties of the job
The post holder will exercise advanced clinical expertise, levels of judgement, discretion and decision making in clinical care.
To work closely with Medical and Nursing staff in both primary and secondary care, to ensure patients receive appropriate investigation, intervention and treatment with minimal avoidable delays, whilst improving the quality of care.
To work autonomously assessing individual patient needs, initiating investigations, determine a differential diagnosis and initiating appropriate holistic, research based treatment with the development of a personalised clinical management plan, including independent prescribing.
To put the patient at the centre of all decisions, reinforcing the concept of a more streamlined patient journey, improving the quality of care.
Monitor and lead improvements to standards of care through, supervision of practice, clinical audit, evidence-based practice, teaching and supporting professional colleagues and the provision of skilled professional leadership.

Clinical Direct Care Practice

Ability to drive is essential.
Lifting and carrying of equipment during home visits.
Potential exposure to infection in patient settings. There is a requirement to follow infection control procedures.
Requirement to use personal computers on a daily basis.
Requirement to be in frequent, direct contact with patients who are acutely ill, infectious and being actively treated for medical conditions.
May be required to directly deal with relatives who are highly distressed and who may exhibit verbal or challenging behaviours. There may also be highly emotive situations to deal with decisions around Respect forms and advanced care planning, preparation for end of life and bereavement support for families.
