We are looking for a Discharge Coordinator to join our Rapid Transfer Team based at QEQM in Margate.
Role Overview
You will work as a key member of the Rapid Transfer Service, aiming to ensure the timely, effective, appropriate, and safe discharge of patients requiring supportive discharge from the acute setting into a community environment. The role involves liaising with hospital, community, GPs, and private facilities to promote safe and robust discharge and transfer of care.
Qualifications and Responsibilities
* A clinical background is essential.
* You will assist the multi-professional team by providing support, advice, and acting as a resource for complex discharge planning, including NHS funded continuing healthcare.
* Work at the interface between hospital and community, offering expert advice on discharge planning and facilitating appropriate discharges through holistic assessments.
* Coordinate with GPs, Community Hospitals, Integrated Care Centres, and other community services, including for patients outside East Kent.
* Promote collaborative working to ensure effective support tailored to patients' social and clinical needs.
* Support the principle of Home First.
