Job Summary
We are delighted to offer an exciting opportunity to join our Community Nursing Team as a Lead Practitioner in our Rapid Response Team based in Burton on Trent. We are looking for an NMC registered professional who can provide strong clinical leadership and support safe and effective care for service users within a busy, community‑focused environment.
Main Duties
* Manage and coordinate a caseload of service users with complex physical health needs, including multiple comorbidities and social or mental health influences, and co‑ordinate MDT meetings when required.
* Take a lead in specialist areas and complex caseload management, providing advice and support to others undertaking community nursing care.
* Formulate plans of care, negotiate the implementation of those plans, and share complex, sensitive, or confidential information as appropriate—for example, explaining diagnosis and treatment plans, discussing disease progression, and addressing prognosis.
* Support staff development by providing mentorship and supervision to pre‑ and post‑registered nurses.
Qualifications – Essential
* Professional NMC Nursing Registration
* Master's degree or equivalent extensive experience in adult nursing
* Current NMC registration (UK)
* Full UK driving licence and vehicle
* Proficiency in IT usage
* Mentorship/Assessor Training (or equivalent experience)
* Experience of providing clinical information and advice to others, including the development of services and staff
* Experience leading a team
* Evidence of continued professional development
* Experience nursing patients with complex needs in the community
* Experience of working effectively across different agencies and with other disciplines
Qualifications – Desirable
* Clinical Leadership experience
* NHS Leadership Academy qualification
* Advanced skills in Excel, Word and PowerPoint
* Leadership qualification (additional)
* Opportunity to contribute to service improvement and quality improvement initiatives
