This is a fantasticopportunity for an enthusiastic, self-motivated nurse to work under theguidance of a Specialist TB Nurse, working with patients across the communityand their homes.

The goals of the service are to identify andreduce the risk of people being newly diagnosed with Tuberculosis and providehigh quality treatment and care for all people with TB
To support the senior nurses in carrying outhealth assessments and prescribed and supportive care.
The post will be based in Wakefield with closelinks to all relevant hospitals, community agencies and the health securityagency.

Main duties of the job
The successful applicant will be part of a proactive teamassisting with the orientation of people new to the country referred for newentrant screening clinics and helping people navigate mainstream healthservices
DUTIES/RESPONSIBILITIES
Thereis a requirement to visit adults, children and families to support with active orlatent TB at home across Wakefield during their treatment, including those withmulti or extensively drug resistance TB
DeliverTB screening clinics, using phlebotomy skills for QuantiFERON testing inaccordance with the Trust Clinical Standards and Guidelines and the NMC Code ofConduct/Guidelines
Recordall patient contacts and activities accurately and contemporaneously usingSystmOne
Participatein health promotion activities on all aspects of TB across health and socialcare, third sector organisations and any other organisation as necessary
Supportand facilitate the direct or video observation therapy of patients undergoingprescribed treatment for TB, in line with TB management guidelines
Travelin a timely manner across Wakefield to undertake home visits and deliverclinics
Participatein large health screening programmes and disease management of patients underthe direction of the senior nurses
Carryout BCG Vaccinations in clinic where appropriate
Workas a member of the Team and where appropriate liaise with regional TB teams tosupport and obtain clinical supervisions
Deputisefor the Specialist Nurse in MDT clinics and meetings as required

Job responsibilities
JOB SUMMARY
To carry out designated duties in a skilled and competent manner in order to provide an appropriate level of skilled nursing care to clients in the community and within the clinic setting.
Establish working relationships within the Primary Care / Asylum Seekers Team and take regular responsibility of managing the caseload in the absence of senior nurses.
KEY RESULT AREAS:
* To support the senior nurses in carrying out health assessments and prescribed care.
* Participate in clinical sessions delegated by Senior Nursing Staff.
* Participate in the training of clinical staff and other professionals.
* Participate in the development of the service under the supervision of senior nurses
CORE DUTIES AND RESPONSIBILITIES
1. Act as the named nurse under the supervision of the senior nurses. This includes the first and continuing care assessment, planning, implementing and evaluation of care given to individuals in their own homes and within the clinic.
2. In the absence of the senior nurses manage the caseload including clients within clinic, monitoring standards of care and deploying and supervising team members as appropriate.
3. Carry out packages of care identified by senior nurses, including liaison with other health professionals and statutory and voluntary agencies in order to improve the quality of life for the individual.
4. To be aware of, and act in accordance with the Trust Clinical Standards and Guidelines and the NMC Code of Conduct and Guidelines.
5. Record all patient contacts and activities accurately and contemporaneously maintaining accurate records and statistical returns as required by the Trust including entering data onto the Trusts systems within the required time frames.
6. Advise patients on health promotion issues, offer support and teach patients, carers and induction centre staff to promote positive help as appropriate to meet identified health needs.
7. Participate in health screening programmes and disease management of patients under the direction of the senior nurses.
8. Participate in all aspects and levels of training in order to consolidate theoretical and practical teaching of staff and learners.
9. Work as a member of the Team and where appropriate liase with Primary Care to support and link with other nursing teams.
10. To have knowledge of and be able to effectively use available resources.
11. To participate in Clinical Supervision on a regular basis with a designated person.
