We are looking for an enthusiastic non-clinical practitioner to join our dedicated team of Health Improvement Practitioners, working within Norfolk in our 4 localities.

This service has been running for over 5 years and has expanded its specialism over that time. We have been providing non-clinical support to the most vulnerable people living in Norfolk.

Supporting patients who frequently attend emergency services in their time of need, helping them gain access to other services and holistically understanding the needs of the patient through behaviour changes, psychological support or advocating and leading the patient to services and gaps in their care.

As a Health improvement practitioner, you will be working within the south locality in Norfolk. Working within a team of 8 that cover the whole of Norfolk. You will have the opportunity to work within a community, with support of clinical staff such as the Matrons to Provide Holistic care to patients, in their own homes, attending appointments, access to workshops/Charity run services and other non- clinical based teams.

Working closely with colleagues from across the organisation, including operational managers, clinicians within the emergency operations centre, the safeguarding team and senior clinical leads, the High Intensity Service User Manager will be responsible for the management of patients with complex needs who present frequently to the ambulance service.

You will be managing your own caseload via SystmOne – triaging patients from the list that the Acute trust sends monthly.

Networking with other services such as Social Services, Norfolk County Council, charities and community teams for drug and alcohol, homelessness etc.

Opportunity to work from Home and in the community on a need basis. Management of own time and caseload.

Work sensitively with compassion and empathy when engaging with patients to support effective communication which facilitates a therapeutic approach to the assessment and management of need, whilst challenging established patterns of accessing healthcare services.

To work closely with Social Care, GPs and other key health professionals to facilitate optimal joint working on safe and effective care for individuals with complex needs.

Actively participate in internal and external multi-disciplinary team / professional's meetings.

Act as a role model in communicating and advocating for the complex needs of patients presenting frequently to the service.

Support and encourage staff to recognise and understand the needs of the patients, provide compassionate, empathetic and patient centred care, and work to.

The post holder will be required to communicate and provide complex information to a wide range of internal and external stakeholders.

Essential skills and competencies

* Experience: History of working with vulnerable and complex patient groups with the care setting.
* Communication: Excellent written and verbal skills to communicate complex information to clients, families, and other professionals.

Interpersonal: Ability to develop good therapeutic relationships and work well in challenging situations.

Organisational: Strong time management and the ability to organize and manage a demanding workload.

Problem-solving: Ability to identify issues, manage challenging situations, and adapt to changing circumstances.

Technical: Computer literacy and the ability to use standard and idiosyncratic measures for outcome evaluation.

Resilience: Ability to remain calm and solution-oriented in a high-stress environment.

Knowledge and experience

Client support: Understanding of social and housing issues that may contribute to high-intensity use.

Risk assessment: Experience in conducting risk assessments.

Evidence-based practice: Knowledge of evidence-based practices and the ability to apply them in a clinical setting.

Service evaluation: Understanding of clinical audit, research, and service evaluation principles.

Therapeutic skills: Ability to develop formulations and create treatment plans, particularly in roles like a High Intensity Therapist where knowledge of psychological therapies is essential.

Personal attributes

Empathy: An empathetic and solution-focused approach to working with clients.

Teamwork: Ability to work both independently and as part of a team.

Adaptability: The ability to adapt to change and develop expertise in the role.

Proactiveness: Taking a proactive approach to troubleshooting and problem-solving.
