Job Summary
We have an exciting opportunity for an experienced Practice Nurse with a qualification in Asthma / COPD or Diabetes to lead a team of nurses and healthcare assistants across both Martock Surgery and South Petherton Medical Centre. The post is a permanent 36‑hour per week role, Monday to Friday, with a pro‑rated salary of £48,645 per annum. Flexibility in days and hours will be discussed at interview, and job‑share and part‑time options will be considered.
Key Responsibilities
* Run a full Practice Nurse clinic, including general nurse appointment cover and chronic disease management.
* Provide safe, clinical decision‑making, assessment and expert care in practice and the home environment.
* Lead and manage the nursing team, ensuring optimal care for the practice population.
* Develop and facilitate upskilling opportunities for staff through accredited courses.
* Support the practice team with operational and clinical governance, audit and quality improvement initiatives.
* Represent the practice in local and professional networks.
Working Patterns
* 36-hour permanent role, Monday to Friday.

Essential Qualifications
* Registered nurse.
* Willingness to learn and acquire new skills.
* Insight and understanding of current issues in nursing and the NHS.
* Evidence of appropriate knowledge base in general practice.
* Basic/advanced certificate of life support.
* Undertaken mentorship training.
* Awareness of national service framework guidelines and health & safety issues.
* Evidence of audit and clinical governance knowledge.
* Knowledge of patient group directions and associated policy.
Desirable Qualifications
* Post‑registration study in topics related to general practice.
* Post‑registration experience in a GP setting.
* Experience using a GP clinical system (EMIS preferred).
* Experience of working with long‑term conditions.
* Experience of managing or leading a nursing team.
* Able to work under pressure and in a changing environment.
* Use of own initiative and decision‑making skills.

* Good interpersonal skills, both verbal and written.
